How do I submit a claim? We provide superbills upon request!
These will list dates of service, diagnostic codes, etc. To receive your superbill, please email us at hello@ascentsportschiro.com. You may then upload the superbill to the website of your respective insurance company. We can also submit your visits on your behalf if you provide all of your insurance and address details. |
